Job Summary:

The Physical Therapist will conduct assessments and reassessments of rehabilitation needs and potential, planning, implementing, and supervising therapeutic programs to restore physical function, improve range of motion, strength, balance, coordination, endurance, or functional ability.

Key Responsibilities:
  • Conduct assessments and reassessments of patients' level of function by applying diagnostic and prognostic procedures.
  • Plan, implement, and supervise individualized therapeutic programs to restore physical function and improve range of motion, strength, balance, coordination, endurance, or functional ability.
  • Coordinate and collaborate with the Rehab Director and other healthcare professionals to ensure integration of the therapeutic program in the plan of care.
  • Guide and teach patients in techniques, exercises, or precautions designed to improve range of motion, strength, balance, coordination, endurance, or functional ability.
  • Observe, record, and report patients' reactions to treatment and any changes in their condition or treatment.
  • Document the extent of patients' involvement in decision-making affecting their care.
  • Participate in continuous assessment and evaluation of the agency through Quarterly Record Review and the Quality Assurance/Improvement Program.
  • Supervise Physical Therapist Assistants (PTAs) treating evaluated patients and be available for phone conferences.
  • Credentialed wound care Physical Therapists participate in wound care according to agency policy and procedure.
Requirements:
  • Graduate of a curriculum in Physical Therapy accredited/approved by the American Physical Therapy Association, or the Committee on Allied Health Education of the American Medical Association, or the Council on Medical Education of the American Physical Therapy Association awarding a Bachelor's degree or higher.
  • Current licensure by the state of Texas to practice as a Physical Therapist.
  • Two years of appropriate experience as a Physical Therapist required. One year of experience in a home health setting preferred.
