Job Title: Nurse Practitioner, I-SNP

Job Summary:

Nascentia Health is seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a key member of our clinical team, you will provide high-quality, patient-centered care to our members in their homes and communities.

Responsibilities:

  • Conduct comprehensive health assessments, diagnose, and treat patients with a wide range of medical conditions.
  • Develop and implement individualized treatment plans, working collaboratively with patients, families, and healthcare teams.
  • Provide preventive and palliative services, including health education, counseling, and support.
  • Order and interpret diagnostic tests, prescribe medications, and refer patients to specialists as needed.
  • Collaborate with healthcare teams to create proactive plans of care focused on early prevention, detection, and timely intervention.
  • Document patient encounters in our electronic medical records system.
  • Participate in case management activities, including building on the principles of geriatric care and Nascentia clinical guidelines.
  • Facilitate medically appropriate and effective plans of care to avoid unnecessary duplication of testing, prolonged hospitalization, and fragmentation of care.
  • Strategize with physician and management teams to enhance cost-effectiveness of services while addressing continuing needs of patients.

Requirements:

  • Current active and unrestricted licensure as an Adult, Family, or Gerontological Nurse Practitioner.
  • 2+ years of hands-on working experience as a Nurse Practitioner.
  • Experience working with complex patient populations, including multiple chronic diseases with diverse psychosocial backgrounds.
  • Possesses or is eligible for DEA licensure and CDS (if required).
  • Basic Cardiac Life Support certification.
  • Ability to travel in the field as needed, with mileage reimbursement available where applicable.
