Cardiac Nursing Specialist

Salary Information:
The estimated salary for this position is $75,000 - $90,000 per year based on experience and location in Richland, SC.
Responsibilities:
This role will have direct and/or indirect supervision of team members with final budget authority, hire/termination authority, performance appraisal responsibility, and disciplinary authority. As a member of management staff at Prisma Health, you will have direct reports. Some of the key responsibilities include:
- Care Coordination: Coordinate care across the continuum for Heart Failure patients in collaboration with cardiologists, advanced practice nurses, and other members of the Multidisciplinary team.
- Patient Education: Develop a therapeutic relationship with patients and their families, facilitating access and utilization of community resources to assist in patient transition.
- Educational Development: Teach and develop curriculum to provide education reinforcement for Heart Failure patients and their families on how to care for themselves at home.
- Data Quality and Performance Improvement: Oversees data quality and performance improvement in the clinic setting.
Requirements:
To be successful in this role, you will need:
- A graduate degree from an accredited school of Nursing program approved by the South Carolina State Board of Nursing.
- A minimum of two years of cardiac nursing experience.
- Required certifications, registrations, and licenses as specified by the department.
