About the Role

Aiken Regional Medical Centers is seeking a skilled and detail-oriented Rehabilitation Outcomes Coordinator to join our team.

As a key member of our interdisciplinary team, you will be responsible for ensuring timely and accurate completion of the Inpatient Rehab Facility Patient Assessment Instrument (IRF-PAI).

You will work closely with healthcare professionals to collect and interpret data, identify trends, and develop strategies to improve patient outcomes.

The successful candidate will have a strong understanding of clinical conditions, excellent communication skills, and the ability to work independently in a fast-paced environment.

We offer a competitive salary of $62,500 - $72,000 per year, depending on experience, plus a comprehensive benefits package.

Responsibilities

  • Coordinate IRF-PAI processes with various hospital departments to ensure timely billing and accurate retention of the IRF-PAI.
  • Collect and interpret data from medical records and coders to complete the IRF-PAI accurately and efficiently.
  • Monitor and input appropriate information into the IRF-PAI, ensuring accuracy and compliance with regulatory requirements.
  • Collaborate with the Program Director and interdisciplinary team members to identify areas for improvement and develop strategies to enhance patient outcomes.
  • Provide education and training to staff on the application of Care Tool completion and chart documentation for GG coding.
  • Assist with data collection and interpretation for performance improvement outcome information.

Requirements

  • Bachelor's degree in nursing or related field required; Master's degree preferred.
  • Current RN license in South Carolina required; QI credentialing exam bi-annually.
  • 2 years of experience in a hospital/clinical setting with knowledge of clinical conditions.
  • Experience in Rehab or skilled nursing setting preferred.
