Job Summary:

We are looking for a compassionate and experienced RN to provide high-quality home health care services to our clients in Immaculata, Pennsylvania. As a Home Health Admission RN, you will be responsible for conducting home visits, assessing patient needs, and developing personalized care plans.


Responsibilities:
  • Conduct home visits to assess patient needs and develop care plans
  • Administer medication, wound care, treatments, and procedures as needed
  • Follow up with and execute doctors' orders
  • Accurately document observations, interventions, and evaluations
  • Travel to cases as assigned

Requirements:
  • Graduation from an accredited and approved nursing program
  • Minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Prior home care experience strongly preferred
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred
