About the Position

We are seeking a caring and dependable Direct Service Provider to join our team. In this role, you will work directly with clients to provide essential support, facilitate independence, and promote social engagement.

  • Develop and implement individualized care plans that meet clients' unique needs and goals.
  • Establish and maintain strong relationships with clients, families, and other stakeholders, ensuring seamless communication and coordination.
  • Maintain accurate records, adhere to regulatory requirements, and ensure a safe and healthy environment.
Key Responsibilities
  1. Deliver quality care and support services, including personal care, transportation, and community activities.
  2. Collaborate with multidisciplinary teams to ensure comprehensive care and service delivery.
  3. Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements and Qualifications
  • Must be at least 18 years old;
  • Hold a high school diploma or equivalent;
  • Be certified in CPR/First Aid and willing to obtain additional certifications as necessary;
  • Have reliable transportation, a valid driver's license, and insurance;
  • Pass a background check, including fingerprinting, and possess excellent communication skills.
