About the PositionWe are seeking a caring and dependable Direct Service Provider to join our team. In this role, you will work directly with clients to provide essential support, facilitate independence, and promote social engagement.
- Develop and implement individualized care plans that meet clients' unique needs and goals.
- Establish and maintain strong relationships with clients, families, and other stakeholders, ensuring seamless communication and coordination.
- Maintain accurate records, adhere to regulatory requirements, and ensure a safe and healthy environment.
- Deliver quality care and support services, including personal care, transportation, and community activities.
- Collaborate with multidisciplinary teams to ensure comprehensive care and service delivery.
- Participate in ongoing training and professional development to enhance skills and knowledge.
- Must be at least 18 years old;
- Hold a high school diploma or equivalent;
- Be certified in CPR/First Aid and willing to obtain additional certifications as necessary;
- Have reliable transportation, a valid driver's license, and insurance;
- Pass a background check, including fingerprinting, and possess excellent communication skills.
