We are seeking a highly skilled Per Diem Pharmacist to join our team in the Pediatric and Women's Health department at Yale New Haven Health. As a Per Diem Pharmacist, you will play a critical role in ensuring the safe and effective use of medications in our patients.
Key Responsibilities- Resolve drug distribution problems to ensure accurate and timely dose delivery to all patients.
- Respond to telephone requests for medication in a professional and courteous manner.
- Interpret clinical and pharmacokinetic lab data to formulate medication monitoring plans.
- Communicate and document problem detection, relevant findings, recommendations, and necessary follow-up.
- Provide drug information to nurses, MDs, and other healthcare professionals.
- Participate in and contribute to educational forums within the department.
- Document medication use variances and adverse drug reactions.
- Effectively demonstrate the principles and conduct consistent with a High Reliability Organization.
To be successful in this role, you will need:
- Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree.
- Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.
- Two years of experience in an acute care hospital pharmacy is preferred for this position.
- Licensure to practice pharmacy in Connecticut (or eligible for reciprocation).
- Excellent communication skills and the ability to work with others.
