Job Summary

We are seeking a highly skilled Clinical Pharmacist II to join our team at Yale New Haven Health. The successful candidate will be responsible for supporting the vision, mission, and goals of our organization, while demonstrating proficiency in all basic clinical pharmacist skills.

Key Responsibilities
  • Customer Service Quality
    • Provide exceptional customer service to patients, families, and healthcare teams, demonstrating CHAMP behaviors at all times.
  • Medication Management
    • Conduct timely and accurate medication order reviews, considering all patient characteristics, including laboratory values and vital signs.
  • Medication Preparation and Distribution
    • Demonstrate competency in pharmacy automation and technology, as applicable to the work area.
  • Medication Safety, Regulatory Affairs, and Quality Assurance
    • Adhere to all departmental policies, procedures, protocols, and guidelines, ensuring a safe and compliant environment.
  • Leadership
    • Provide education and training to pharmacy staff members, students, and residents, as well as healthcare team members, promoting excellence in patient care.
  • Financial/Volume Results
    • Contribute to departmental cost-reduction and revenue generation initiatives, driving financial success.
Qualifications

To be successful in this role, you will need:

  • Education
    • Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
  • Experience
    • Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is preferred, but not required.
  • Licenses and Certifications
    • Current Connecticut Pharmacist License, or eligibility for pharmacist licensure in the state of Connecticut.
  • Skills
    • Demonstrate in-depth knowledge of pharmacology and therapeutics, as well as understanding of pharmacy systems of drug distribution and formulary management.
