Job Title: Registered Nurse

About the Role:

As a Registered Nurse with UPMC Home Healthcare, you will have the opportunity to work in a dynamic and supportive environment, collaborating with a team of specialized nurses, rehabilitation therapists, social workers, dieticians, aides, and administrative staff to provide personalized patient care.

Responsibilities:
  • Provide direct patient care, evaluating outcomes, and adjusting the nursing care process as needed to ensure optimal patient care.
  • Establish and implement individualized plans of care in collaboration with the physician, patient, and interdisciplinary team members.
  • Coordinate case management activities within an interdisciplinary healthcare team to ensure appropriate delivery of services to patients.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Supervise and teach other nursing personnel, including LPNs and Home Health Aides, as needed.
Requirements:
  • Current licensure as a Registered Professional Nurse in the state where the facility is located or in a state covered by a licensure compact agreement.
  • Graduate nurses must complete licensure examination within six months of hire or within one year of graduation, whichever comes first.
  • Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.
