As a Nurse Practitioner in our Hospice division, you will play an essential role in supporting patients and their families during a significant phase of their life journey.

  • This is a full-time, salaried position with a schedule from Monday to Friday during daytime hours.
  • Applicants must hold valid Pennsylvania CRNP and Registered Nurse licenses, possess a valid Pennsylvania driver's license, and have reliable internet access.

In your role as a Nurse Practitioner, you will be responsible for conducting medical evaluations and palliative care visits necessary for certifying patients' eligibility for the Medicare Hospice Benefit after six months of hospice care. You will document visits in the patient's clinical records and complete required forms for face-to-face encounters. Additionally, you will provide medical care and palliative interventions for hospice patients as needed, collaborating closely with the Medical Director, Hospice Director, the interdisciplinary team, and fellow Nurse Practitioners.

Qualifications:

Education
  • A master's degree or higher is required for Nurse Practitioners to obtain certification in Pennsylvania.
  • Advanced studies in anatomy, physiology, chemistry, psychology, nursing, and microbiology are essential. Master's programs typically combine classroom learning with clinical practice.
  • National certification necessitates passing a comprehensive examination.
Experience
  • A minimum of two (2) years of experience as a Nurse Practitioner is required.
  • A solid understanding of palliative and hospice care programs is essential.
Licensure, Registration, and/or Certification
  • Must be currently licensed and nationally certified as a Nurse Practitioner by the Commonwealth of Pennsylvania's Board of Nursing (CRNP).
  • Proof of completion of 45 hours of advanced pharmacology education is required, which must be part of an advanced program.
  • A valid driver's license and reliable, insured transportation are necessary.
  • Successful completion of background checks is mandatory.
  • Reliable internet access is required.
