Job Title: Vna Rn Case Manager

Mass General Brigham is seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager. This is an exciting opportunity to make a meaningful difference in the lives of our patients and their families.

Job Summary:

The VNA RN Case Manager is responsible for providing high-quality, patient-centered care to individuals and families in their homes. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:

  • Provide skilled care and case management to patients and families in accordance with the State Nurse Practice Act and accepted professional standards of practice.
  • Communicate and document observations and assessments, and promote and maintain the health of individuals through direct care, teaching, counseling, and appropriate rehabilitative measures.
  • Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide education and support to patients and families to promote health and wellness, and to empower them to take an active role in their care.
  • Participate in quality improvement initiatives to ensure that care is delivered in a safe and effective manner.

Requirements:

  • Graduate of an approved, accredited school of professional nursing required; BSN preferred.
  • Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
  • Current CPR certification required.
  • Minimum of one (1) year Medical/Surgical nursing experience required; home care/hospice experience preferred.
  • Demonstrated excellent verbal and written communication skills and strong interpersonal skills required.
  • Demonstrated organizational and time management skills required.
  • Current drivers license, a good driving record, and reliable transportation required.
