Job Summary:The VNA RN Case Manager is responsible for delivering skilled care and case management services to patients and families in accordance with the State Nurse Practice Act and accepted professional standards of practice. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Responsibilities:- Provide skilled care and case management services to patients and families, including assessment, planning, implementation, and evaluation of care.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support.
- Document patient information and care plans accurately and thoroughly.
- Graduate of an approved, accredited school of professional nursing required; BSN preferred.
- Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
- Current CPR certification required.
- Minimum of one (1) year Medical/Surgical nursing experience required; home care/hospice experience preferred.
- Demonstrated excellent verbal and written communication skills and strong interpersonal skills required.
- Demonstrated organizational and time management skills required.
- Current driver's license, a good driving record, and reliable transportation required.
