Job Title: Practical Nurse

We are seeking a skilled and compassionate Practical Nurse to join our team at Professional Case Management. As a key member of our healthcare team, you will provide high-quality nursing care to our clients, working under the direction of a registered nurse.

Responsibilities:
  • Provide nursing care as defined in the nurse practice act for a Practical Nurse, under the direction of a registered nurse.
  • Contribute to the assessment of client health status, collecting, reporting, and recording objective/subjective client data.
  • Observe client conditions or changes in condition, recognizing and reporting changes to the case manager and physician as directed.
  • Assist in the identification of healthcare problems and in formulating a list of client needs/problems.
  • Contribute to setting realistic short and long-term goals for assigned clients.
  • Follow the plan of care developed by a registered nurse, participating in the development of a written plan of care.
  • Recognize, understand, and respect cultural, spiritual, and religious beliefs, needs, and rights to choose.
  • Assist in ensuring access to community resources, participating in the identification of priorities and reviewing and revising the plan of care.
  • Carry out functions taught in an approved Practical Nurse program, assisting the client with ADL's, encouraging self-care.
  • Provide direct care, comfort measures, emotional support to patients who have a stable or predictable condition.
  • Administer prescribed treatment and medications, except by intravenous route (except IV-certified Practical Nurses per advanced preparation).
  • Document prescribed interventions and responses, assisting and providing reinforcement with client/family/other teaching.
  • Maintain a safe and effective care environment for clients and coworkers, consulting/seeking guidance from an RN and others as indicated.
  • Initiate standard emergency procedures until an RN or physician is available, applying principles of asepsis and infection prevention and control.
  • Participate in the development, revision, and implementation of policies and procedures, recognizing and responding appropriately to potentially unsafe situations.
  • Evaluate client response to interventions, evaluating the effectiveness of common treatments/tasks, documenting and communicating outcomes of care given.
  • Assist with the collection of evaluation data, contributing to the modification of the plan of care, collaborating with other healthcare professionals in providing client care.
  • Communicate verbally and through complete and accurate documentation in the clinical record, seeking education/training or assistance with areas of concern.
  • Perform the job in compliance with Agency policies and procedures and professional and community standards, accepting responsibility for the professional nursing role.
  • Follow applicable state and federal laws and agency standards, maintaining confidentiality in all aspects of the job.
  • Refrain from discussing client or staff identities, conditions, or circumstances in public or with others who do not have a legitimate interest in the person.
  • Participate in the development, implementation, and evaluation of the Agency Performance Improvement Program and pertinent PI activities, attending meetings and continuing education offerings as required.
  • Perform other related duties and responsibilities as deemed necessary.
