Job Title: LPN - Licensed Practical Nurse

First Choice Health Centers is seeking a skilled Licensed Practical Nurse (LPN) to join our team in our outpatient primary care clinic. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

The LPN will be responsible for performing various nursing tasks, including taking vital signs, administering medications, collecting samples, documenting patient information, and collaborating with other healthcare professionals to ensure optimal patient outcomes.

Responsibilities:
  • Provide direct and individualized nursing care to patients based on the application of the nursing process
  • Maintain accurate medication inventory listings and oversee the CT Vaccine Program in accordance with Center policies and requirements
  • Accurately document patient information in the electronic health record, including assessments, vital signs, specimen collections, interventions, evaluations, and responses
  • Manage prescriptions and medication refills for providers in accordance with the Nurse Practitioner's scope of practice
  • Perform periodic meaningful use documentation, including depression screening, sexual history, alcohol and substance abuse history, and assessment
  • Work with medical assistants to ensure safe and effective patient care is rendered in accordance with the medical assistant's scope of practice
  • Interact with outside agencies to manage patient care outside the Center
  • Monitor task lists to respond in a timely manner, follow up on overdue orders, and monitor the status of critical referrals
  • Adhere to all patient safety standards and infection control guidelines
  • Assist the care team to ensure smooth office operation and delivery of excellent service through teamwork
Requirements:
  • A valid LPN license in the state of Connecticut
  • BLS Certification
  • Experience with an electronic health record system
  • Excellent communication and interpersonal skills
  • A positive attitude and willingness to learn
