As a key member of the Cherokee Nation healthcare team, the Registered Nurse - Ambulatory Care Specialist will provide exceptional care management services in the ambulatory healthcare setting. This role requires a strong understanding of Cherokee Nation standing orders, policies, and guidelines.
Key Responsibilities- Provide health care services to individuals eligible for services in accordance with the self-governance compact and funding agreement between the Cherokee Nation and the United States.
- Complete pre-visit planning for scheduled patients and communicate patient needs and recommended treatment to the staff on their team.
- Maintain and update patient panels, identify high-risk subpopulations, and track performance measures monthly.
- Participate in ongoing quality improvement activities using the Model for Improvement.
- Provide patient education and self-management support, triage patients, and document observations and assessments.
- Review patient medical records to determine eligibility for grants and other programs.
- Plan, develop, and implement assurance monitors for designated grants or programs.
- Ensure validity, quality, and timeliness in grant and other program reporting submissions.
- Track and follow up on patients receiving abnormal screening results.
- Associates degree from a 2-year college or technical school in Nursing.
- Must possess a license as a Registered Nurse (RN) from the Oklahoma Board of Nursing (OBN) or possess a multistate RN license accepted by the OBN.
- Must be certified in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
- Must meet and maintain pre-employment and periodic background investigation and adjudication for child care.
The Registered Nurse - Ambulatory Care Specialist will work in a fast-paced healthcare environment, occasionally exposed to fumes or airborne particles and toxic or caustic chemicals. The noise level in the work environment is usually moderate.
