Position Summary:
The Licensed Practical Nurse (LPN) plays a vital role in delivering personalized care tailored to each individual's requirements. This position involves conducting patient interviews, documenting medical histories, and assessing physical conditions. The LPN is responsible for obtaining and monitoring vital signs, including pulse, blood pressure, temperature, and respiration, while providing essential routine care for patients.

Key Responsibilities:
1. Deliver individualized care for clients, adhering strictly to their care plans while promoting dignity and independence.
2. Monitor and report clients' vital statistics, communicating any abnormal findings to the case manager and physician.
3. Administer medications, assist with feeding, provide oxygen therapy, and perform ostomy care as outlined in the client's care plan.
4. Collaborate with other healthcare professionals, including Physical, Occupational, and Speech Therapists, to complete established routines.
5. Uphold infection control protocols at all times, reporting potential communicable diseases and ensuring adherence to infection control practices.
6. Accurately document all nursing actions and communicate effectively with family members, case managers, and physicians regarding the client's progress and any changes in their medical condition.
7. Manage the ordering of supplies and medications to maintain adequate stock levels.
8. Ensure all patient needs are met in accordance with physician directives.
9. Monitor incidents involving clients, ensuring timely and accurate completion of incident reports and informing case managers.
10. Maintain medical equipment and report any malfunctions to the appropriate service for repair.
11. Perform additional duties as assigned.

Qualifications:
1. Completion of post-secondary education necessary for licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
2. Understanding of individual service plans, client rights, and health requirements.
3. Valid and current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing.
4. CPR certification required.
5. Strong interpersonal skills, including customer service, patience, empathy, and compassion.
6. Comprehensive knowledge of patient care policies and regulations.
7. Familiarity with symptoms and indicators of improvement for various medical conditions.
8. Willingness to travel between facilities as needed.
