Job Summary

We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients in the Nassau, NY region.

You will apply medical techniques and principles under the supervision of a physician and care for a diverse set of patients with chronic diseases.

Key Responsibilities
  1. Provide high-quality care to primary and palliative patients in and around Nassau, NY
  2. Assess patients' health by performing physical examinations, obtaining, updating, and studying medical histories, and creating the best plan for care
  3. Effectively communicate with office staff and clinical operations across various internal platforms
  4. Provide high-quality, patient-centered care to a panel of patients
  5. Attend quarterly medical staff meetings
  6. Develop impressions, diagnoses, and treatment plans
  7. Foster trust and consult with patients, families, and caregivers during visits
  8. Order diagnostic testing as medically necessary
  9. Prescribe medications, supplies, and appropriate medically necessary interventions
Requirements

We are seeking a Board-Certified Advanced Provider with a valid NP or PA certification in the State of New York.

Strong clinical skills, a positive demeanor, and strong people skills are essential.

Ability to confidently problem-solve and operate independently is required.

1-2 years of primary care experience with geriatric patients, with hospice experience highly preferred but not required.
