Job Summary:
We are seeking a highly skilled and detail-oriented Treatment Team Coordinator III to join our team at the Texas Department of Aging & Disability Services. As a key member of our treatment team, you will play a vital role in coordinating and facilitating treatment planning meetings, gathering and compiling information, and drafting comprehensive individualized treatment/recovery plans.

Key Responsibilities:
  • Attend treatment team meetings and gather input from all disciplines to inform treatment planning.
  • Compile and synthesize large amounts of information into clearly written and concise treatment/recovery plans.
  • Enter data into the electronic medical record, ensuring accuracy and timeliness.
  • Develop and maintain a tracking system to ensure timely completion of treatment/recovery plans.
  • Perform administrative duties, including coordinating schedules, maintaining files, and participating in mandatory training.
  • Serve as a liaison/contact person for program staff, patients, and centralized rehabilitation staff.

Requirements:
  • Knowledge of office practices and administrative procedures.
  • Knowledge of HIPAA and confidentiality rules.
  • Proficient knowledge and skills in computer use and written language, including spelling, punctuation, medical, and psychiatric terminology.
  • Ability to synthesize large amounts of information, coordinate team members, and work under pressure.
  • Effective communication and interpersonal skills.
