We are seeking a skilled Case Manager to join our interdisciplinary team at Rehabilitation Hospital of Rhode Island. As a Case Manager, you will play a vital role in the care and recovery of our patients, working closely with healthcare professionals to develop and implement individualized treatment plans.
Key Responsibilities- Collaborate with the healthcare team to identify patient needs and develop comprehensive treatment plans
- Coordinate patient care conferences to ensure effective communication and goal achievement
- Document patient progress and outcomes in accordance with CMS guidelines
- Communicate with patients and caregivers to provide updates and support
- Identify and address potential barriers to care and develop solutions to ensure successful patient outcomes
- Current licensure as a Registered Nurse, Social Worker, or Healthcare Professional
- Certification in Case Management or Rehabilitation Nursing preferred
- Minimum of 2 years experience in an inpatient setting, preferably in acute or rehabilitation hospital
- Effective communication and interpersonal skills
- Basic computer skills and proficiency in Microsoft Office
- Competitive hourly rate of $32.27 - $48.40
