Nurse Practitioner

We are seeking a compassionate and skilled Home Health Registered Nurse to join our team at Mederi Caretenders - Fairview Heights. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and promoting a culture of caring, belonging, and trust.
Key Responsibilities:- Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other healthcare team members.
- Make initial and comprehensive nursing evaluations, determine primary focus of care, develop the plan of care within state-specific guidelines, and submit accurate, complete, and timely documentation.
- Regularly evaluate and re-evaluate patient nursing needs, ensuring they meet home health eligibility and medical necessity guidelines.
- Initiate, develop, implement, and make necessary revisions to the plan of care in collaboration with physicians and other healthcare professionals.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
