Home Health Registered Nurse Opportunity

We are seeking a compassionate and skilled Registered Nurse to join our team at Mederi Caretenders in Fairview Heights, IL. As a Home Health Registered Nurse, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

This is a full-time, part-time, or PRN opportunity to provide high-quality nursing care to patients in their homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Conduct comprehensive assessments of patients to identify their nursing needs and develop individualized care plans
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education
  • Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes
  • Document patient care and progress in a timely and accurate manner
Requirements

To be successful in this role, you will need to have:

  • A current RN licensure in the state of Illinois
  • Current CPR certification
  • A reliable vehicle and a valid driver's license
