PRN Physical Therapist Job Description

Pinnacle Home Care Inc. is seeking a dedicated and compassionate PRN Physical Therapist to join our team of skilled home health care providers.

Job Summary

The PRN Physical Therapist will provide physical therapy services to patients in their own homes, assessing their conditions, developing personalized treatment plans, and delivering therapy to help improve mobility, reduce pain, and restore function.

Responsibilities
  • Conduct initial and ongoing comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Establish and revise treatment plans in consultation with physicians as necessary.
  • Assist in development and implementation of interdisciplinary client care plans as it pertains to Physical Therapy.
  • Evaluate patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests, recording and reporting any adverse reactions to treatment.
  • Treat clients to relieve pain, develop or restore functional and maintain maximum performance using physical means such as exercise, massage, heat, light, and electricity.
  • Teach, supervise, and counsel families and clients in the physical therapy program, instructing them in care and use of adaptive devices such as wheelchairs, braces, crutches, canes, prosthetic, and orthotic devices.
  • Record all evaluation data, treatments, and clients' responses to therapy intervention, as well as quality assurance data as directed.
  • Prepare evaluations of clients' conditions and perform periodic reassessments.
Requirements
  • The ability to drive and access clients' homes, which may not be routinely wheelchair accessible, is required.
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
  • On occasion, may be required to bend, stoop, reach, and move client weight up to 250 pounds, lift, and/or carry up to 30 pounds.
  • Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Must be licensed or registered by the State.
  • The Physical Therapist in charge of PT services must have a home care experience and at least one year of supervisory experience (2 years preferred).
  • Must have one year of experience in an acute/sub-acute setting.
