Physical Therapist PRN Job Description

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Physical Therapist to join our team of skilled home health care providers. As a Physical Therapist PRN, you will have the opportunity to provide quality care to our patients in their own homes, making a meaningful difference in their lives.

Job Summary:

As a Physical Therapist PRN, you will be responsible for assessing patients' conditions, developing personalized treatment plans, and delivering therapy to help improve mobility, reduce pain, and restore function. You will work closely with our interdisciplinary team to ensure high-quality care and patient safety.

Responsibilities:

  • Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement treatment plans in consultation with physicians and revise them as necessary.
  • Assist in the development and implementation of interdisciplinary client care plans as it pertains to Physical Therapy.
  • Treat patients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light, and electricity.
  • Teach, supervise, and counsel patients and their families in the physical therapy program.
  • Record all evaluation data, treatments, and patients' responses to therapy intervention in a timely manner.
  • Prepare evaluations of patients' conditions and perform periodic reassessments.
  • Participate in case conferences, physical therapy staff meetings, agency activities, and in-service programs as necessary.

Requirements:

  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Licensed or registered by the State.
  • At least one year of experience in an acute/sub-acute setting.
  • Home care experience and at least one year of supervisory experience (2 years preferred).
