Job Title: Referrals Coordinator

Overview:

LifeLong Medical Care is seeking a highly organized and detail-oriented Referrals Coordinator to join our team at our West Berkeley Health Center. As a key member of our patient-centered care team, you will play a vital role in providing exceptional patient care and ensuring seamless referrals to specialty services.

Responsibilities:

  • Process and communicate referral requests from providers, ensuring timely and accurate responses.
  • Request urgent appointments as needed and verify current insurance information.
  • Coordinate with HMOs, PPOs, Medi-Cal/Medicare, and other special funding sources to obtain authorizations for services.
  • Follow up on all referrals and maintain accurate tracking in Epic and i2i.
  • Collaborate with the Health Information Technician to ensure medical report information is received and placed in the chart.
  • Perform other duties as assigned by the Supervisor.

Qualifications:

  • Commitment to providing primary care services for underserved populations, with demonstrated ability and sensitivity in working with diverse patient communities.
  • Strong organizational, administrative, and problem-solving skills, with excellent time management and prioritization abilities.
  • Ability to work effectively under pressure in a positive and friendly manner, with flexibility and adaptability to change.
  • Excellent interpersonal, verbal, and written communication skills, with the ability to present information to others in a clear and concise manner.
  • Ability to work independently with minimal supervision, using professional judgment and diplomacy.
  • Experience working in a team-oriented environment, with a strong understanding of how one's work intersects with other departments and partner organizations.

Requirements:

  • High school diploma or GED.
  • One year of administrative experience in a medical or human services setting.
  • Experience with electronic health records and proficiency in Microsoft Office.

Preferred Qualifications:

  • College degree in a related field.
  • Community or public health experience.
  • EPIC experience.
  • Knowledge of medical terminology and Medi-Cal and Private HMO Insurance plans.
  • Bilingual English/Spanish/Arabic.
