Job Summary

The Wright Center Medical Group is seeking a highly skilled and compassionate Certified Registered Nurse Practitioner to join our team in the ID Clinic. As a key member of our healthcare team, you will provide high-quality patient care, working closely with Infectious Disease Attending physicians to deliver evaluation and follow-up services for outpatients.

Key Responsibilities
  • Manage patients receiving outpatient intravenous antibiotics, patients with HIV or hepatitis C, and patients with latent and active tuberculosis.
  • Perform comprehensive assessments, diagnose, order, conduct, supervise, and interpret diagnostic and laboratory tests, prescribe pharmacologic and non-pharmacologic treatments in the direct management of acute and chronic illness and disease.
  • Work closely with the inpatient infectious disease team, coordinate with other specialties, skilled nursing and rehabilitation facilities, and home care companies to facilitate safe transitions of care from inpatient to outpatient or long-term care settings.
  • Provide women's health and wellness services, including breast and cervical cancer screening/PAP tests, and coordinate a respective Clinical Quality Improvement (CQI) project.
  • Provide and monitor continuity of care between patient visits, evaluate and manage emergency psychosocial counseling and support to patients and families, and make community referrals as necessary.
  • Identify treatment-related toxicities and initiate medical interventions, evaluate patient intervention outcomes, and demonstrate expertise in symptom-management.
  • Monitor and adjust patient medications and treatments according to protocols, manage telephone calls related to emergency situations, symptom management, and psychosocial and family concerns.
  • Provide continuity of care across settings (home, home health, skilled nursing, hospice, and rehabilitation), exercise HIPAA confidentiality and security measures at all times.
  • Participate in quality improvement reviews and activities in the clinical setting, perform and document comprehensive medical and psychosocial health histories and examinations on patients.
  • Collaborate with physician in managing acute and long-term medical needs of patients, interview and advise patients regarding health and illness prevention, recommend resources to meet patient and family needs, and instruct patients and families regarding medications and treatment instructions.
Requirements
  • Current license to practice as a Nurse Practitioner (CRNP) in Pennsylvania.
  • CPR Certification and BLS.
  • DEA number from the United States Drug Enforcement Administration.
  • Ability to function both independently and in collaboration with other healthcare professionals.
  • Previous experience working in a mental health specialty a plus.
  • Extensive HIV/AIDS experience preferred.
