We are seeking a skilled Admissions Registered Nurse to join our team at HealthEcareers - Client. As an Admissions Registered Nurse, you will play a critical role in the admissions process for incoming hospice patients, providing education and support to patients, families, and caregivers.

Key Responsibilities
  • Admissions Process: Determine patient eligibility for hospice care through visual contact, interviewing, and medical history review.
  • Initial Assessment: Complete the initial assessment to address patient/family care needs.
  • Admission and Transfer: Complete the admission and transfer process, explaining hospice services, insurance benefits, and required paperwork.
  • Patient Education: Provide education and support on symptom management and end-of-life care for patients, families, and caregivers.
  • Communication: Communicate patient/family needs and significant care plan issues to the patient's assigned Registered Nurse Case Manager and on-call staff.
  • Consultation: Consult with physicians for orders and assist in obtaining necessary supplies, equipment, medications, and services.
Requirements
  • Education: Graduate of an accredited school of nursing and current Registered Nurse (RN) licensure in the State of Michigan.
  • Experience: Two years of recent clinical experience preferred, but recent nursing graduates are also encouraged to apply. Homecare or hospice experience desired.
  • Clinical Judgment: Sound clinical judgment and knowledge of pain symptom management with end-of-life care.
