We are currently seeking a Senior Care Coordinator to enhance our communities in Texas and Oklahoma.

This position plays a crucial role in leading the community's Clinical Department, ensuring adherence to safe nursing practices and regulatory compliance. Key responsibilities include managing the resident care department, promoting a person-centered care philosophy, and overseeing clinical initiatives. This role reports directly to the Divisional Director of Resident Care.

Key Responsibilities:

General Duties

  • Possess a strong understanding of clinical practices and operational protocols in assisted living and memory care environments.
  • Implement departmental policies and operational procedures.
  • Provide direct care to residents and support for their families.
  • Identify and address high-risk resident issues.
  • Deliver training and educational resources to the clinical team.
  • Evaluate the effectiveness of systems and recommend improvements.
  • Ensure compliance with organizational policies.
  • Conduct audits for compliance and develop corrective action plans.

Resident Care and Compliance

  • Direct resident care policies in accordance with regulatory standards.
  • Stay informed on regulatory changes and implement necessary adjustments.
  • Monitor regulatory outcomes and provide recommendations for enhancements.
  • Collaborate with legal and compliance teams on regulatory matters.

Risk Management and Safety

  • Analyze incidents and accidents to mitigate risks.
  • Ensure comprehensive investigations and prompt reporting of incidents.
  • Educate staff on infection control measures.
  • Guide the management of infectious diseases and outbreaks.
  • Track key performance indicators to identify quality trends.
  • Supervise the resident care department.

Occupancy Development

  • Ensure adherence to resident assessment protocols.
  • Address resident complaints and grievances effectively.
  • Engage in interdisciplinary teams to meet resident needs.

Leadership Development

  • Assist in the recruitment process for clinical roles.
  • Support the onboarding and continuous development of clinical personnel.
  • Foster nursing leadership and clinical competencies.
  • Collaborate on staffing and budgetary planning.
