Key Responsibilities:
- Evaluate patients' functional status and occupational therapy needs, developing individualized plans of care to promote independence and wellness.
- Provide evidence-based occupational therapy interventions, including adaptive equipment recommendations and home assessments, to support patients' rehabilitation and recovery.
- Collaborate with healthcare professionals, family members, and caregivers to ensure seamless care coordination and optimize patient outcomes.
- Maintain accurate and timely documentation of patient progress, communicating effectively with the healthcare team and stakeholders.
Requirements and Qualifications:
- Clinical experience as an Occupational Therapist, preferably in a home health or geriatric setting.
- Licensure in good standing, meeting state requirements for occupational therapy practice.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
- Ability to work independently and collaboratively as part of a dynamic team.
Compensation and Benefits:
- A competitive annual salary range of $80,000 - $110,000, commensurate with experience.
