Community Navigators play a crucial role in eliminating obstacles to healthcare by linking uninsured residents of Delaware to programs that enhance healthcare affordability. They engage with the community to provide education on accessible health coverage options through personal interactions and outreach initiatives, while also offering support with the application and enrollment processes for various State and Federal insurance programs. These include Medicaid, Children's Health Insurance Program (CHIP), the Health Insurance Marketplace, Health Care Connection (HCC), and Screening for Life (SFL). Additionally, Community Navigators are tasked with connecting community members to a medical home.

KEY RESPONSIBILITIES

  1. Complete the online Navigator certification during the initial training phase and maintain it annually.
  2. Adhere to all required Navigator responsibilities as specified by CMS.
  3. Safeguard personally identifiable information (PII) to uphold confidentiality in compliance with HIPAA regulations.
  4. Create a personalized work plan detailing outreach activities and enrollment objectives, and develop checklists to help achieve these goals.
  5. Foster professional relationships and collaborate with community partners to establish a referral system.
  6. Conduct educational sessions on health insurance literacy and the significance of primary and preventive care at community events and through individual interactions. Distribute informative materials at outreach events and other public venues.
  7. Assist Delaware residents in completing applications for Marketplace, Medicaid, HCC, and SFL, as applicable.
  8. Prepare timely and detailed reports for all consumer interactions and outreach activities.
  9. Acquire extensive knowledge of healthcare resources available within the community.
  10. Participate in all mandatory meetings and training sessions.

MINIMUM QUALIFICATIONS

  1. High school diploma or GED.
  2. At least six months of experience with Microsoft Office applications (Word, Excel, Outlook).
  3. Proficiency in both written and spoken English and Spanish or Haitian Creole, or English and another language prevalent among our patient demographic.
  4. Possession of a valid driver's license, reliable transportation suitable for carrying outreach materials, and the ability to travel independently between various work locations, including outreach events.
  5. Capability to lift a minimum of 35 pounds.
  6. Willingness to work evenings and/or weekends as necessary.
  7. Readiness to deliver presentations at public gatherings.
  8. Consent to undergo a criminal background check in accordance with state and federal regulations (previous offenses will be evaluated on a case-by-case basis and will not automatically disqualify applicants).
