Position Overview:
The Healthcare Case Coordinator will assist and support the medical team with daily operational tasks associated with this role. Responsibilities include the coordination of patient services, management of specialty referrals, and providing support to patients in their care while identifying relevant resources. The Healthcare Case Coordinator will engage in collaboration and outreach to external providers to ensure smooth transitions of care, facilitate patient education, and assist in accessing necessary resources. This role is essential in promoting timely access to required care, ensuring comprehensive continuity of care, and enhancing patient well-being.
Qualifications:
- Education: High school diploma or GED required; an associate degree or relevant certification preferred (e.g., CMA, NCMA, or pursuing a degree).
- Experience: At least two (2) years of experience in the healthcare field.
- Licenses and Certifications: CPR or First Aid certification required within six months of employment.
- Specialized Skills: Must possess a solid understanding of medical procedures.
- Preferred Experience: One to two years of experience in case management.
Key Responsibilities:
- Document patient history and personal information accurately.
- Measure and record vital signs, such as blood pressure.
- Assist healthcare providers during patient examinations.
- Administer injections or medications as directed by the provider and in accordance with state regulations.
- Schedule patient appointments efficiently.
- Prepare and send blood and bodily fluid samples for laboratory testing.
- Input patient information into the Electronic Health Record (EHR) system.
o Ensure direct patient care aligns with clinical policies and meets patients' needs.
o Update and revise patient healthcare plans as necessary.
o Demonstrate knowledge of medical care, including diagnostic procedures, medications, symptoms, and treatment-related therapies.
o Assess and determine the needs of patients effectively.
o Review and evaluate the effectiveness of individualized care plans.
o Collaborate with providers as needed to enhance patient care and performance improvement initiatives.
- Develop care plans with clear objectives, goals, and actions tailored to meet patients' needs.
- Implement and monitor care plans to ensure the appropriateness and effectiveness of services.
- Provide referrals to suitable community resources and other healthcare facilities; facilitate access and communication when multiple services are involved; monitor activities to ensure services are delivered effectively and meet patient needs.
o Refer any patient whose health status has deteriorated to the provider and follow up on the referral.
- Conduct patient consultations.
- Clinically assess clients' adherence to medication.
- Administer nursing care as prescribed by the provider.
Additional Duties:
- Perform Medical Assistant duties as required.
- Prepare reports, memos, letters, and other documents using relevant software.
- Carry out general office tasks, including ordering medications and maintaining the records management database.
- File and retrieve medical documents, records, and reports as needed.
- Coordinate travel arrangements for staff as necessary.
