Job Summary

The Eventus WholeHealth team is seeking a highly skilled Primary Care Healthcare Professional to provide exceptional care to patients in skilled nursing facilities and assisted living facilities.

This role requires a collaborative approach, working closely with physicians, nurses, and other healthcare professionals to deliver high-quality patient care.

Responsibilities

  • Conduct comprehensive assessments of patients' health status
  • Develop and implement treatment plans
  • Order and interpret diagnostic tests
  • Prescribe medications and other treatments
  • Provide patient education and counseling
  • Collaborate with physicians and other healthcare professionals to provide coordinated care
  • Maintain accurate and complete medical records
  • Participate in quality improvement initiatives
  • Adhere to all applicable laws, regulations, and standards of practice

Qualifications

  • Master's degree in nursing or physician assistant studies
  • Current state licensure as a nurse practitioner or physician assistant
  • Certification as a nurse practitioner or physician assistant
  • Experience in primary care, preferably in a skilled nursing facility or assisted living facility
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Ability to maintain confidentiality and adhere to ethical standards
