Job Summary: We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team in providing exceptional hospice care to patients and families in Charleston, Berkeley, and Dorchester Counties.
Key Responsibilities:
- Maintain a specified case management load and complete initial and ongoing assessments of patients' physical and psychosocial status.
- Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
- Coordinate care among IDG members and other healthcare providers to ensure continuous assessment and implementation of the IDG plan of care.
- Evaluate and coordinate the need for services provided to patients and families in accordance with the IDG plan of care.
- Direct and supervise the services provided by LPNs, Hospice Aides, and Homemakers assigned to patients and families.
Requirements:
- Graduate of an accredited school of nursing with a current RN license in the state of South Carolina.
- Twelve months of accumulated experience as a registered nurse within the last five years or evidence of current clinical knowledge.
- One year of hospice care experience and one year of precepting or training experience preferred.
- CPR certification and ability to provide proof of valid driver's license and auto liability insurance.
