Join Us as a Hospice RN Case Manager

We are seeking a compassionate and empathetic Hospice RN Case Manager to provide personalized care to patients and their families during their time of need. As a key member of our team, you will act as a primary liaison between physicians, patients, and families to ensure the highest level of care and support.

Responsibilities:

  • Provide direct patient care services and supportive care to the patient's family
  • Empower patient's caregivers through education on care for their loved ones
  • Manage and collaborate with an interdisciplinary group to provide comfort, peace, and dignity
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
  • Maintain all assigned patient clinical records

Requirements:

  • RN license in the state of employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred
  • Current driver's license and automobile insurance
  • Current CPR certification
