Overview:

Angel Care Hospice and Palliative is actively seeking a dedicated and experienced Hospice LVN to deliver compassionate end-of-life care to our patients and their families. This role is vital in the Los Angeles County regions, including various communities. The ideal candidate should possess a solid understanding of hospice principles and be willing to travel within designated areas.

Our patients receive care in their homes, assisted living facilities, or nursing homes throughout Southern California. As a Hospice LVN, you will communicate directly with patients and their caregivers to coordinate visit schedules effectively. Our nursing team collaborates in a supportive interdisciplinary environment to ensure the highest standards of care and services are provided to our patients. We strive to support families during a challenging and emotional period.

Key Responsibilities:

  • Coordinate with assigned patients to schedule visits as per the prescribed frequencies.
  • Engage with the hospice team regarding patient needs and updates.
  • Provide education and emotional support to patients and their families.
  • Monitor patient responses to interventions and modify care plans as necessary.
  • Foster effective working relationships with the hospice team throughout the case management process.
  • Proactively advocate for patients' end-of-life goals through meaningful interactions.
  • Maintain precise and timely documentation of patient progress notes.
  • Promote quality and cost-effective care interventions and outcomes.
  • Adhere to professional standards in accordance with established protocols and regulations.

Qualifications:

  • Demonstrated experience in hospice care.
  • Strong communication skills.
  • Ability to work independently and collaboratively within a team.
  • Excellent organizational and time management abilities.
  • Robust clinical knowledge to manage complex patient needs.
  • Compassionate and empathetic demeanor.
  • Current LVN license.
  • CPR Certification.
  • Valid driver's license and reliable transportation.
