We are seeking a highly skilled and experienced Licensed Clinical Social Worker to join our team at Body Fit Plus Inc. as a correctional facility social worker.

Job Summary:

The successful candidate will be responsible for providing high-quality behavioral healthcare services to clients of all ages in a correctional facility setting. This is a full-time contract position with the potential for extension.

Key Responsibilities:
  • Assessment and Treatment: Conduct thorough assessments of patients to identify their needs and develop individualized treatment plans.
  • Communication: Maintain accurate and comprehensive patient documentation, and ensure timely and correct care is provided.
  • Collaboration: Work closely with primary health care providers, specialists, psychiatrists, dependency programs, and other healthcare professionals to provide comprehensive care.
  • Case Management: Gather health history through patient and/or family member interviews, and maintain patient follow-ups for referrals.
  • Documentation: Maintain patient documentation in a prompt and comprehensive manner.
Requirements:
  • Twelve (12) continuous months of experience within the last three (3) years rendering direct patient care in a public or private institution.
  • Valid Photo ID, Driver's License, or Passport.
  • Possess current Basic Life Support (BLS) certification from an American Heart Association.
  • Valid California Social Worker license.
