Job Title: Mental Health Case Manager

Segue Inc. is a leading non-profit organization dedicated to delivering exceptional mental health services to the residents of Jackson and Hillsdale counties in Michigan.

Job Summary

We are seeking a passionate and empathetic individual to join our team as a Mental Health Case Manager. As a key member of our DREAMS Case Management program, you will play a pivotal role in assisting individuals with mental health challenges in accessing vital services, including entitlements, medical care, social support, education, and more.

Responsibilities
  • Assist individuals with mental health challenges in accessing vital services, including entitlements, medical care, social support, education, and more.
  • Advocate and support individuals to achieve their highest level of recovery and functioning.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with healthcare providers, social services agencies, and community organizations to ensure comprehensive care.
  • Provide education and support to clients and their families on mental health issues and available resources.
  • Document client progress and maintain accurate records.
Requirements
  • Verifiable Bachelor's degree in Social Work, Human Services, Criminal Justice, or Nursing (or licensed Registered Nurse - RN).
  • Qualified Mental Health Professional (QMHP) with specialized training or a minimum of one year of experience working with individuals with mental illness.
  • Proficient typing skills (minimum 40 WPM).
  • Proficiency in using iPads, Apple cell phones, and other technologies preferred.
  • Effective multitasking abilities in a fast-paced environment.
  • Strong organizational and time management skills.
  • Excellent communication and teamwork abilities.
  • Previous experience with Electronic Medical Records (EMR) system preferred, but not required.
  • Valid driver's license, vehicle insurance, and reliable transportation required.
