Job Title: Mental Health Case Manager/AdvocateJob Summary

Segue Inc. is seeking a compassionate and determined Mental Health Case Manager/Advocate to join our Assertive Community Treatment (ACT) program. As a key member of our team, you will work closely with adults who have a history of severe mental illness, providing support to help them remain safely in their communities and reduce the risk of hospitalization or the need for intensive psychiatric services.

Responsibilities
  • Provide case management services to adults with severe mental illness, including assessment, planning, and coordination of care.
  • Develop and implement individualized treatment plans to meet the unique needs of each client.
  • Work collaboratively with clients, family members, and other healthcare professionals to ensure comprehensive care.
  • Monitor client progress, identify areas for improvement, and make adjustments to treatment plans as needed.
  • Document client interactions, progress, and treatment plans in a timely and accurate manner.
  • Participate in team meetings and contribute to the development of program goals and objectives.
Requirements
  • A verifiable Bachelor's degree in Social Work, Human Services, Criminal Justice, or Nursing is REQUIRED. Alternatively, being a licensed Registered Nurse (RN) will also be considered.
  • Qualified Mental Health Professional (QMHP) with specialized training or at least one year of experience treating or working with individuals with mental illness.
  • Proficient typing skills with a minimum typing speed of 40 WPM
  • The ability to use IPads, Apple cell phones, and other technologies is preferred.
  • Ability to effectively prioritize and manage multiple tasks in a fast-paced environment.
  • Excellent organizational and time management skills.
  • Strong communication and teamwork abilities.
  • Previous experience using an Electronic Medical Records system (EMR) is preferred but not required.
  • A valid driver's license, vehicle insurance, and reliable transportation are necessary for this position.
