Job Summary

Atrium Health Navicent is seeking a skilled Social Work professional to join their team as an Acute Care Navigator. This role will play a critical part in facilitating patient care, providing ongoing care management services, and enhancing the quality of clinical outcomes and patient satisfaction.

Key Responsibilities
  • Participate in rounds on the patient care unit with the attending physician and other members of the healthcare team to ensure collaboration and consistency in moving the patient's care to estimated date of discharge.
  • Assess patients to determine their discharge planning and/or post-acute transition needs, developing a discharge plan that addresses physical, functional, social, and psychological status, cultural and language needs, caregiver resources, and available benefits.
  • Assign the appropriate care pathway based on clinical feedback from the physician and diagnosis-DRG, ensuring coordination of services among the patient's physicians, specialists, community agencies, and vendors.
  • Assess progress toward goals and identify barriers to meeting goals, preparing and maintaining appropriate documentation of patient care and progress within designated systems.
Requirements
  • Bachelor's degree in Social Work, Master's preferred.
  • Three years of recent experience in acute care, home health, case management, discharge planning, or care management.
  • Discharge planning experience in a healthcare setting preferred.
  • Experience with IT solutions such as electronic health records, learning management, or disease/care management systems a plus.
