About the Role

ChristianaCare is seeking a highly skilled Social Work Specialist to join our team in Wilmington, Delaware.

Job Summary

The successful candidate will be responsible for providing comprehensive social work services to high-risk populations, addressing social determinants of health and promoting overall well-being.

Key Responsibilities
  • Conduct thorough assessments of clients' psychosocial and environmental factors to inform care plans.
  • Develop and implement individualized care plans in collaboration with clients and multidisciplinary teams.
  • Collaborate with community and government agencies to secure resources and services for clients.
  • Provide education and support to clients on navigating healthcare systems and accessing community resources.
  • Advocate for clients' rights and interests, addressing barriers to health and well-being.
  • Document client interactions and progress in a timely and accurate manner.
  • Participate in multidisciplinary case reviews to optimize outcomes and share knowledge with colleagues.
  • Supervise undergraduate or graduate students enrolled in social work programs, as required.
Requirements
  • Masters in Social Work (MSW) degree from an accredited graduate school of social work.
  • Current or recent experience in a healthcare setting, with a focus on care planning and community resource coordination.
  • Case Management Certification through the Commission for Case Management (CCM) or equivalent.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.
