**Job Description:

We are seeking a highly skilled Healthcare Services Coordinator to join our team at the State of Colorado. As a healthcare services coordinator, you will be responsible for coordinating and managing healthcare services for our residents, ensuring they receive timely and effective care.

**Estimated Salary Range:** $60,000 - $75,000 per year, depending on experience.

**Responsibilities:

  • Coordinate and manage healthcare services, including scheduling appointments and arranging transportation.
  • Communicate effectively with healthcare professionals, patients, and families to ensure seamless care coordination.
  • Develop and implement individualized care plans, working closely with patients and their families to meet their unique needs.
  • Maintain accurate records and reports, including patient progress and treatment outcomes.

**Requirements:

  • Bachelor's degree in Healthcare Administration or related field.
  • Minimum 2 years of experience in healthcare administration or a related field.
  • Excellent communication and interpersonal skills.
