Job Title: Home Health Registered Nurse

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse. As a Home Health Registered Nurse, you will provide high-quality patient care to individuals in their homes, focusing on skilled nursing visits and case management.

Job Summary:

The ideal candidate will have at least 2 years of experience working with medical/surgical patients in a variety of settings, including home health care, skilled nursing facilities, and hospitals. You will be responsible for assessing patients' needs, developing and implementing care plans, and providing education to patients and their families.

Responsibilities:
  • Provide skilled nursing visits to patients in their homes, focusing on medical/surgical care, wound management, and medication management.
  • Assess patients' needs and develop individualized care plans to meet their unique needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide education to patients and their families on disease management, medication use, and self-care techniques.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Current RN license in the state of practice.
  • At least 2 years of experience working with medical/surgical patients in a variety of settings.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to travel to patients' homes and work in a fast-paced environment.
