Job Title: Home Health Registered Nurse

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse. As a Home Health Registered Nurse, you will provide high-quality patient care to individuals in their homes, focusing on medical and surgical patients with complex needs.

Job Summary:

The Home Health Registered Nurse will be responsible for:

  • Providing skilled nursing visits to patients in their homes, focusing on medical and surgical patients with complex needs.
  • Assessing patients' physical and emotional needs, developing and implementing individualized care plans, and evaluating patient progress.
  • Collaborating with healthcare teams to ensure seamless care transitions and coordination.
  • Managing patient medications, wound care, and other medical interventions as needed.
  • Documenting patient information accurately and thoroughly, adhering to regulatory requirements and company policies.
Requirements:

To be successful as a Home Health Registered Nurse, you will need:

  • At least 2 years of recent experience working with medical and surgical patients in a home health or skilled nursing setting.
  • A current RN license in the state and a valid US citizenship or green card.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong assessment and critical thinking skills, with the ability to prioritize patient needs and develop effective care plans.
  • Proficiency in electronic medical records and other healthcare software.
