Licensed Practical Nurse

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care to residents in a comfortable and therapeutic environment. The ideal candidate will assist individuals in their recovery and daily living activities.
Key Responsibilities:- Patient Care: Deliver direct patient care under the supervision of Registered Nurses (RNs) and physicians, including administering medications, monitoring vital signs, and assisting with daily living activities.
- Rehabilitation Support: Collaborate with rehabilitation therapists to implement and support individualized care plans, encouraging residents to reach their maximum level of function.
- Documentation: Maintain accurate and up-to-date patient records, including assessments, interventions, and progress notes, ensuring compliance with facility policies and regulatory standards.
- Team Collaboration: Work closely with nursing staff, therapists, and other healthcare professionals to provide comprehensive care and develop a supportive environment for residents.
- Patient Education: Educate residents and their families about health conditions, treatment plans, medications, and rehabilitative exercises.
- Infection Control: Adhere to infection control protocols to ensure the safety and well-being of residents and staff.
- Emergency Response: Respond promptly to emergency situations, implementing appropriate intervention measures as needed.
Qualifications:
- Licensure: Current and valid LPN license in the state of practice.
- Experience: Previous experience in long-term care, rehabilitation, or similar healthcare.
- Education: Graduate of an accredited practical nursing program.
- Skills: Excellent communication and interpersonal skills, strong organizational and time-management abilities, proficient in electronic medical records (EMR) and basic computer skills, and a compassionate and patient-centered approach to care.
Physical Requirements:
- Ability to lift and assist patients as necessary.
- Stand, walk, and move for extended periods.
- Bend, stretch, and reach as required in patient care tasks.
Work Environment:
- Fast-paced nursing and rehabilitation facility.
- May require shift work, including nights and weekends.
