Gentiva Hospice offers a unique opportunity to join a dedicated team of healthcare professionals who make a difference in people's lives every day. As a Hospice Registered Nurse / RN Case Manager, you will report directly to the Administrator, Executive Director or Patient Care Manager Senior and maintain primary control and professional management of each patient. You will act as primary liaison between physician, patient/family and hospice team, providing direct patient care services and supportive care to the patient's family.
The successful candidate will demonstrate flexibility and composure while supporting others in a time of crisis. A current driver's license and automobile insurance are required, as well as CPR certification. This role involves traveling to patients' homes and hospitals, so reliable transportation is essential.
