About the Role

We are seeking a skilled Hospice Clinical Social Worker to join our team at AccentCare, Inc. As a key member of our healthcare team, you will play a vital role in providing compassionate care to our patients and their families.

Key Responsibilities
  • Provide emotional and social support to patients and their families in a hospice setting
  • Assess and address the psychosocial needs of patients and their families
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient progress and maintain accurate records
Requirements
  • Master's Degree in Social Work or related field from an accredited institution
  • Valid license to practice as a clinical social worker in the state of operation
  • Medicare/Medicaid/Insurance specialty preferred
  • Ability to travel to all business locations
