Job Summary

We are seeking a highly skilled Registered Nurse to work in a remote setting, providing comprehensive care management services to patients with end-stage renal disease (ESRD). The successful candidate will have 1+ years of nursing experience in case management or care management, preferably coordinating care across multiple settings.

Responsibilities:

  • Conduct comprehensive assessments to identify medical, behavioral, pharmaceutical, and social needs of patients
  • Create and implement personalized care plans to address identified needs and improve patient outcomes
  • Collaborate with patients, nephrologists, PCPs, and other healthcare professionals to ensure seamless care transitions
  • Provide education and support to patients on self-management of their renal condition
  • Manage patients during periods of transition to minimize avoidable readmissions
