Job Title: Community Case Manager- Mental Health

Job Summary:

Elevate Services is seeking a Community Case Manager- Mental Health to provide support and guidance to individuals working to establish and/or sustain a solid foundation for their recovery from mental health and/or substance use disorders. The successful candidate will assist clients in achieving independence and stability in their own residence, while maintaining a professional and personable demeanor.

Key Responsibilities:

* Develop, monitor, and support client treatment goals and service plans, including obtaining necessary services
* Provide direct client/staff interaction to develop and support improved lifestyle changes, including transportation of clients if necessary
* Participate in staffing with Washington County Human Services and Elevate
* Excellent communication skills, including written documentation and verbal skills
* Empathetic and influential intervener in the patterns of client behavior

Qualifications:

* A minimum of a Bachelor's degree in a related field is required
* Experience in adult care, and mental health and alcohol and other drug abuse services
* Experience and training as required under HFS 36 relative to psychosocial rehabilitation services is required
* Knowledge of and experience providing services under HFS 36, Comprehensive Community Services for persons with Mental Health and Substance Use Disorders is preferred
* Excellent communication and organizational skills
* Strong interpersonal skills
* Knowledge of mental health diagnoses and treatment, CBT, and/or psychopharmacology is preferred
* Knowledge of patient rights and privacy laws
* Valid Wisconsin Driver's License and eligibility for employer's automobile insurance coverage is required
