Join Our Team as a Home Health RN Case Manager

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at Preceptor Home Health & Hospice. As a Home Health RN Case Manager, you will play a vital role in providing high-quality care to patients in their homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Key Responsibilities:
  • Conduct thorough assessments of patients' needs and develop comprehensive care plans to ensure optimal outcomes.
  • Collaborate with healthcare professionals, patients, and families to coordinate care and services.
  • Provide education and support to patients and caregivers on disease management, medication adherence, and self-care techniques.
  • Monitor patients' progress, making adjustments to care plans as needed to ensure continuity of care.
  • Document all interactions, assessments, and care plans in a timely and accurate manner.
Requirements:
  • Current licensure as a Registered Nurse in the State of Wisconsin.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Good organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Willingness to travel to meet patient needs, with a valid driver's license and reliable transportation.
