Job Summary:
We are seeking a skilled Registered Nurse Case Manager to join our team at Integrity Placement Group. As a Hospice Nurse Case Manager, you will play a vital role in providing high-quality patient care and support to individuals and their families in the Gwinnett/Dekalb County area of Georgia.
About the Role:
- As a Hospice Nurse Case Manager, you will be responsible for maintaining a caseload of approximately 12-14 patients, providing individualized care and support to meet their unique needs.
- You will work closely with patients, families, and healthcare teams to develop and implement comprehensive care plans that promote comfort, dignity, and quality of life.
- Responsibilities will also include back-up call coverage to ensure seamless patient care.
Requirements:
- Registered Nurse licensure in the State of Georgia
- At least 1 year of experience in Hospice or Palliative Care, with a preference for those with 2 years of in-home patient care experience
- Familiarity with electronic medical record (EMR) systems, particularly KanTime, is a plus

- A competitive annual salary range of $75,000-$85,000
