KEY RESPONSIBILITIES:

  • Execute designated nursing tasks, aligned with your qualifications and training within the scope of LPN practice, including but not limited to: tracheostomy care, colostomy care, enemas, douches, catheterizations, dressing changes, and phlebotomies.
  • Provide education to patients and families as per the plan developed by the RN.
  • Assess and document vital signs and other significant observations.
  • Track and report patient intake and output, noting any critical changes immediately.
  • Observe, evaluate, and document the physical and mental status of patients, reporting any variations.
  • Administer prescribed medications as directed by the physician and under the guidance of the RN, ensuring accurate documentation.
  • Participate in interdisciplinary patient case discussions, staff meetings, and training sessions as required.
  • Collaborate effectively with patients, families, and healthcare professionals to coordinate care.
  • Be available for on-call duties, including weekends and after-hours visits, as needed.
  • Uphold confidentiality at all times due to access to sensitive patient information.
  • Maintain consistent attendance and demonstrate flexibility to meet the agency's needs.
  • Adhere to all Medicare, Medicaid, and HIPAA regulations and requirements.
  • Follow all organizational policies, procedures, and standards.
  • Perform additional responsibilities as assigned.
POSITION REQUIREMENTS &

QUALIFICATIONS:

  • Graduate from an accredited practical nursing program.
  • Licensed as a vocational/practical nurse in the relevant state.
  • Minimum of one (1) year of clinical nursing experience, preferably in intensive care, rehabilitation, or medical-surgical settings.
  • Experience in home health care is preferred.
  • Capability to conduct home visits and provide nursing care in a residential setting.
  • Proficient in completing necessary documentation accurately and promptly.
  • Strong interpersonal, verbal, and written communication skills essential for fulfilling job responsibilities.
  • Must possess reliable transportation, a valid driver's license, and appropriate automobile insurance; or the ability to utilize public transportation.
